Kids in Mask Photo Contest - Cottonwood Pediatrics

I give Cottonwood Pediatrics permissions to use, publish, and republish for purposes of advertising, public relations and any other lawful use of the photograph(s) attached with or without my name. I, the parent or legal guardian, hereby waive any right that I (and minor(s)) may have to inspect or approve the copy and/or finished product or products that may be used in connection therewith. I also waive any right, claim or interest to control the use of the photograph(s) without any compensation. I am the lawful author/copyright/authorized party of the images/works submitted. Submissions will be subject to review by Cottonwood Pediatrics. Winners will be notified via text - message and data rates may apply. Prizes must be claimed in person within 30 days of notification. Limited quantities.
